- Local physician leadership and an administrative support team
- Physician practices control 75% of the ACO governance
- The Chief Medical Officer, Board, and Committee chair positions are controlled by members of the physician leadership team and are representatives of the participating practices
- 70% of shared savings revenue will be distributed to participants and 30% percent to the sponsoring hospital
- Administrative costs are funded by the sponsoring hospital and recouped through shared savings prior to distribution
- The hospital will provide admission, discharge and transfer feeds to ACO participant practices and will facilitate transitional care coordination follow-up visits with the PCP
- The hospital will assist with the reduction of avoidable emergency room and inpatient utilization, as well as readmissions and ambulatory care sensitive condition admissions
- The ACO will provide training and operational support to promote quality improvement and successful quality reporting
- The ACO will provide staff to manage post-acute care utilization and align patients with community resources
